3.  Verification of Teaching Obligation - For Program Completers
(link to teaching verification)      

a.  Print Verification Form      
b.  Beginning of School Year, Fill out Sections I and II (Parts A, B, and C) and send to U.S. Department of Education (address on first page)      
d.  End of School Year, Fill out Section I on page 2 and all information on pages 4 and 5 and send to U.S. Department of Education
